Filling in form 1

Section 1

You don’t have to provide a reference, but if you do, we will use it whenever we contact you.

Section 2

Enter the full name, address and postcode of the person applying, underlining the surname. If there is more there 1 applicant, give details for every applicant, continuing on a separate sheet if necessary.

If you apply in your company's name, tell us where it is incorporated, including the province or state if in Canada or the United States of America (USA).

Section 3

Enter the title of your invention here.

Section 4

You must supply a contact address, which we call an address for service, which may be in the European Economic Area, to which all our letters should be sent.

If you appoint someone for example, a patent attorney to deal with your application, give details here.  If you leave this section blank, we will write to you at the first address given in section 2.

Section 5

Only fill this section in if you are claiming priority from one or more previous applications. In most circumstances, you can claim priority only from one or more applications filed within the last 12 months.

Section 6

Only fill in this section if you made an earlier application and we have told you that your earlier application contained more than one invention.

Section 7

If you answer No to the first question, you need to fill in form 7 (32Kb). You also need to fill in form 7 if one of the applicants is a company or organisation or if you answer Yes to the second question.

Section 8

If you send the application fee with this form, we will carry out a preliminary examination. You may prefer to pay the application fee with form 9A (32Kb).

Section 9

Enter the number of each type of page you are sending - do not count copies of the same page or document.  If you give us details of a previous application instead of filing a description, you will not be able to add information to what you filed originally in that application.

Section 10

Tell us here if you have included anything else with your application.

Section 11

You and the other applicants (if any) must sign and date the form.

Section 12

It will help us to sort out any queries more quickly if you can provide the name and daytime phone number of someone we can contact.
