Filling in the application form

Section 1

This is optional, but we will use it in correspondence if included.

Section 2

Is the application in your name or your company's name? If you apply in your company's name, tell us where it is incorporated, including the state if it is in the United States of America.

If your address is outside the United Kingdom, you must also provide an address in the European Economic Area (EEA)  in section 3.

If you have applied to us before you will have been given an ADP (Automatic Data Processing) number. This is our reference number for you. Please include your ADP number, if you know it.

Section 3

You do not have to appoint an agent or attorney, but if you do please record their details here.  You can also provide an address for service where you want your correspondence to go (which must be in the EEA)

If you leave this section blank, we will write to you at the address given in section 2 as long as it is in the EEA.

If you know it, please include your agents ADP number.

Section 4

Briefly describe what the design is applied to.  Write the fee code 'T' for textiles or 'O' for other products.

Section 5

If you are claiming priority from an earlier application made in another country, provide details in this section. You must apply within six months of the application date of the earlier registration request.

Section 6

Only fill in this section if you are not the person or company named in the priority application in section 5.

Section 7

Only fill in this section if you made an earlier application and we have told you that your earlier application contained more than one design.

Section 8

You or your agent must sign and date the form.

Section 9

It will help us to sort out any queries more quickly if you can provide the name and daytime phone number of someone that we can contact.
